Description The Nurse Coordinator for the Heart Failure Disease Management portion of the Advanced Heart Failure Comprehensive Care Center coordinates care for our patients being evaluated for general heart failure management, as well as those who may be moved into advanced heart failure therapies workups. This role will coordinate between inpatient and outpatient departments, and manage patient panels including recommending diagnostic testing, lab testing, new and investigational heart failure…
